An appointment can only be done made as a referral from your vet who needs to complete a Faxback Referral (appointment request) Form, and send it with a referral letter and relevant clinical history. Once the SLVH have received the correct paperwork they will contact you directly to make an appointment with Clare and inform your veterinary surgeon. After the consultation a report will be sent to both you and your vet. If you are an owner wishing to be referred you must arrange this via your own veterinary surgeon.
